Patients

We would like to welcome you to our practice and introduce to you some guidelines that may be helpful in the future.

Emergencies: If you have an emergency after hours or on weekends, please call our office and the answering service will connect you with the physician on call, or proceed to St. Elizabeth East Emergency Room. In case of life threatening emergencies, please call 911.

Medicine Refills: Please call for medicine refills during office hours, at least 5 days before you run out. Medicine refills are not called in after hours. Patients may need office visit prior to the refills. You can also obtain refills from your primary care physician.

Pain Medicines: It is office policy not to prescribe chronic pain medications. You will need to contact your primary care physician for chronic pain medications.

Cancellation Policy: Please give us at least 3 business days’ notice if you will be unable to make a scheduled appointment, to avoid a charge of $50.

Payment Policy: In order to provide the best care possible, it is necessary that all payments be collected at the time of service. We will be glad to bill your insurance. But, if you have a co-payment, unmet deductible, or are responsible for a percentage of the charges, please bring a form of payment with you to your appointment. We accept cash, personal checks, MasterCard and Visa.
